Connections to the BMS
1) Determine which sense wire goes where on the battery and then solder . 2) Determine what goes on the 4 poles on the other end of the BMS (I assume it is charge and discharge wires). 3) Install on battery back... not certain what the "best practice" is here. The BMS is small enough to fit on the side of the battery.
B+ is positive from the cell pack. B- is the negative from the cell pack. P- is the BMS negative to your controller .
Yes, you can charge/discharge through the same connector . Charge is typically less current than discharge peaks and therefore discharge usually has thicker wiring via the BMS to protect against over discahrge of the pack.
Removing a swollen battery can be hazardous , but leaving a swollen battery inside a device also poses risks. To prevent potential device and bodily harm, a device should not operate with a swollen battery.

You definitely need flow-through ventilation around your batteries due to the very real risk of hydrogen explosion.

A LiPo fire is a chemical fire. Always keep a Class D fire extinguisher nearby your battery charging/discharging and storage area. Yes, you need to take precautions while charging them as the charger might fail to cutoff properly and when discharging them under a high current load. Other than that they are as safe as any other type battery. Considering the fire risk, it makes sense to charge and store lipo batteries outside your house .
Centralized: a single controller is connected to the battery cells through a multitude of wires . Distributed: a BMS board is installed at each cell, with just a single communication cable between the battery and a controller.
C- is where you connect the charger to Charge the battery. P- is where you connect the controller to Power it from the battery . If the BMS has a common charge/discharge (charger/controller) port, then you only need to use whichever single wire / pad goes to that port for the Charge input and Power output connections.
